How to File a Claim
There are critical documents which must be filled out when filing for a claim. For personal workplace injuries, an Employees Claim (C3) form must be obtained, completed, and filed. For accidents that tragically result in the death of an employee, the next of kin will fill out a (C62) form instead, then must also obtain a (C64) from the physician who treated the departed before the loss of life, and a (C65) from the funeral home director. These forms can be obtained and completed in the following manners:

- In Person: At a local Customer Service Sentence or District Office
